Change of Details

Use the form below to inform the school as to any changes that need to be applied to your child's records.  It is important that the details that the school holds for your child are up to date at all times.

Example of changes that the school should be informed of are:

  • change of address
  • change of parent / carer contact details
  • change of GP
  • changes to dietary or medical information
